| Dermatophyte test media inoculated from affected areas of hair, scab, skin, beaks, claws, feathers, toenails for laboratory identification. Indicate date of inoculation of culture. |

| Specimen should be taken after first cleaning the area with 70% alcohol or a non-medicated, non-fungicidal soap. Specimen may be obtained by plucking hair with scabs, scraping nail bed or skin with scalpel blade |
