1) COMPLETE ONLINE SUBMISSION FORM

  1. Click here to log in to the CoreOne Online Client Portal to complete the OVDL submission process.
  2. When finished, SAVE, and print the PDF from the prompt at the top left of the screen.   
  3. Mail your completed online submission form with the sample(s) to the lab.

2) OBTAIN MAILING SUPPLIES

The OVDL accepts specimens shipped through all commercial delivery services, including UPS and FedEx, and the U.S. Postal Service (U.S. Mail). UPS and FedEx are recommended for perishable specimens. While clients can ship specimens in their own containers, the OVDL is also able to provide mailers for shipping specimens. For your convenience and savings, we offer package mailers with the option of prepaid postage on shipping of specimens through UPS (with guaranteed overnight delivery, Monday-Friday). Clients using the pre-paid UPS service are not charged additional shipping by UPS, instead charges will be reflected on the invoice from the OVDL

Please note: When reusing boxes that have been used for other shipments, it is essential that ALL prior labels are removed or covered. As package sorting becomes more automated, any stray labels or marks (on any of the six sides) can cause packages to be routed inappropriately. This causes violations for the delivery service provider and results in fines against the OVDL. Delivery services can end contracts with clients who have repeated violations.

3) PACKAGE SAMPLES

Specimen and packaging requirements differ depending on the nature of the test ordered. Packaging specimens (with minimal likelihood that pathogens are present OR for diagnosis of an infectious disease) requires sufficient inner AND outer packaging. 5) CONFIRM SPECIMEN ARRIVAL

If using the OVDL pre-paid UPS mailers, retain your half of the billable stamp as it includes your UPS tracking number. Space is provided on that receipt for you to make notes for your records if you wish.

If you would like to confirm receipt of your specimens, you can view your test submission status via our CoreOne Online Client Portal.  Alternatively, you can call us at 541.737.3261.

To cancel a test that has been requested. You can cancel a test via the CoreOne Online Client Portal or contact the general laboratory number at 541.737.3261. If a test is already in progress, charges will apply.

6) RECEIVE RESULTS

Please see Reporting Options and Information for more details on methods for receiving laboratory reports and how to make changes if needed.

If you have questions about results, you are welcome to contact the laboratory, Monday through Friday, at 541.737.3261. Please have the OVDL account number, owner's name, date specimens were sent, accession number, and test(s) requested available. In the interest of privacy and continuity of care, results will be shared only with the client of record (the clinic or person paying the bill).

All samples and specimens submitted become the property of the OVDL and will not be returned unless specific arrangements are made and approved in advance by the OVDL management. Samples, specimens, and related test and diagnostic results may be used for teaching and research purposes. Test and diagnostic results will be shared with appropriate state or federal agencies as required by law.

If you are a new client please complete the New Client Registration Form to create an account with the OVDL. After completing the online form, you will receive two documents via DocuSign that require the primary account holder to electronically sign. These documents are required whether using the online client portal or paper forms for submissions.

Note: Samples will not be tested until a new account is set up and all documents have been electronically signed by the primary account holder.

A submission form should accompany each submission. If using the portal, you can print the submission form once the submission process is completed. Or if you would like to use a paper submission form, our most current forms are available below.  While the OVDL accepts both forms of submission, use of the CoreOne Online Client Portal offers many benefits, such as real time testing status, the ability to view invoices, statements, and make secure online payments. 

NOTE: If completing a submission form by hand, please write as legibly as possible to expedite data entry.

CAN I STILL USE OLD SUBMISSION FORMS?

The CoreOne Online Client Portal is the preferred method for submissions. We still accept paper submission forms, but prefer you discard any outdated forms, and use the current form from the link above.
